HHS Announces Grants for Communities to Fight Chronic Disease

Chronic diseases are currently the leading causes of death in the United States and make up 75% of health care costs.

In an effort to stem the rise of chronic diseases, the U.S. Department of Health and Human Services (HHS) has announced another round of available grant funding aimed at helping communities implement projects proven to reduce chronic disease.

Community Transformation Grants represent an enormous investment and commitment to health and community prevention, but more federal funding is still needed (and was recommended for appropriation by the Senate HELP Committee) to ensure that all communities have access to safe, equitable health outcomes.
